This is a new patient appointment for someone who has not been to this office previously or more than 2 years ago. It consists of an exam and usually a treatment.
Please provide us with your insurance information and date of birth so that we can verify your insurance before your arrival. In rare cases a referral from your Primary Care Provider is needed before you can see us. You can reply to the confirmation email with your information. Thank you.
